Pediatric / Congenital CPB · Educational reference

Pediatric CPB Calculator

Flow conversion, indexed oxygen transport and predicted post-dilution hematocrit, with source links to the AmSECT 2025 standards.

Educational reference · Not clinically validated. This tool implements arithmetic associated with selected standards, not a complete protocol or an AmSECT-endorsed calculator. No universal flow, DO₂i or transfusion thresholds are supplied. Institutional protocols and patient physiology determine clinical targets.

Clinical Evidence and original sources ↓

Patient measurements

The BSA selector reuses the existing BSA calculator; AmSECT does not mandate Haycock or any other selected equation. Costeff is restricted to 1.5–100 kg and is not a neonatal-specific formula. The Hct section requires weight only.

1. Flow conversion

AmSECT Standard 10.2

Convert an independently selected flow target. No rate is preselected or recommended.

2. Oxygen delivery and consumption

AmSECT Standard 9.2 · Hb coefficient 1.36

Use effective patient flow and representative, contemporaneous arterial and mixed venous samples. Pump flow may include shunt or vent flow; this tool does not subtract it automatically. Circuit or central venous samples may not represent mixed venous blood.

3. Predicted post-dilution Hct

AmSECT Standards 13.1 and 15.3

Initial complete-mixing estimate from RBC-volume conservation. This arithmetic implementation is derived; the guideline requires prediction but does not prescribe or validate this specific equation. Bleeding, UF, cell loss and redistribution are excluded.

Inputs are held in browser memory, not saved by this calculator. Copy/share includes the inputs. Editing any input invalidates all prior results. Inputs and results clear after 15 minutes of inactivity. Numeric limits are error guards, not validated clinical ranges.

AmSECT Standards and Guidelines for Pediatric and Congenital Perfusion Practice (2025)

The uploaded document is the official standards text. The 2026 companion publication describes the 2025 update; it is a separate publication, not a 2026 replacement of the source standards. Standards 9.2 and 10.2 support oxygen-transport and flow calculations. Standards 13.1 and 15.3 address prime composition and predicted dilutional Hb/Hct. Standard 15.4 leaves acceptable Hct to institutional protocol.

Formula details

Q (mL/min) = selected index (L/min/m²) × BSA × 1000, or selected rate (mL/kg/min) × weight (kg)

CI (L/min/m²) = effective patient flow (mL/min) / 1000 / BSA

CaO₂ = 1.36 × Hb × SaO₂/100 + 0.0031 × PaO₂; CvO₂ = 1.36 × Hb × SvO₂/100 + 0.0031 × PvO₂

DO₂i = 10 × CI × CaO₂; VO₂i = 10 × CI × (CaO₂ − CvO₂); units mL O₂/min/m²

DO₂ = Q × CaO₂ / 100; VO₂ = Q × (CaO₂ − CvO₂) / 100; Q in mL/min

Extraction ratio (%) = 100 × (CaO₂ − CvO₂)/CaO₂; undefined when CaO₂ = 0

BV = weight × selected blood-volume coefficient; predicted Hct (%) = (BV × patient Hct (%) + RBC volume × product Hct (%)) / (BV + cell-free prime + RBC volume)

Standard 9.2 prints its CI-based consumption equation as VO₂. Because CI is indexed, its dimensional result is VO₂i; this calculator distinguishes indexed and total values explicitly. Saturation inputs are percentages converted to fractions. The Hct equation and extraction ratio are derived mass-balance/Fick arithmetic, not validated prediction equations supplied by AmSECT. The BSA formulas are reused from the existing BSA tool and are not mandated by this guideline. All intermediate values remain unrounded.

Calculated oxygen consumption requires representative paired samples and an appropriate effective-flow estimate. Shunts, collateral flow and non-steady-state conditions can make the Fick estimate unreliable. No adequacy, AKI-risk or transfusion classification is inferred. Use the original standards and local protocols for monitoring, gas exchange, blood-prime testing and clinical decisions.
